What do we offer?
Reconnect is a community based early intervention program that is federally funded by the Department of Social Services to work with Aboriginal and Torres Strait Islander young people aged 12 – 18 years who are homeless or at risk of homelessness.
Reconnect works with young people and their families to support and stabilise their housing situation/s. Reconnect further supports young people to provide linkages and engagement opportunities with family, education, and employment within their local communities. Reconnect staff use a range of interventions and strategies with young people and their family such as:
- Assessment and goal setting
- Case Management
- Advocacy
- Counselling (individual and family)
- Mediation
- Group Work
- Outreach
- Practical Support
- Brokerage
- Therapies (individual and family)
- Collaboration and referral pathways with other services (specialist, community stakeholders and networks)
